a. Overview

Menopause is the point in time when a woman’s menstrual periods stop permanently. After this time a woman can no longer get pregnant. Often, menopause is accompanied by uncomfortable symptoms such as hot flashes. Menopause typically occurs around age 51, but may vary between a woman’s 40s or later in her 50s. Menopause is a normal part of life and happens to everyone. However, the discomfort associated from the symptoms may be helped. During this time, a woman has changes in her levels of estrogen and progesterone.

In many women menopause causes bone loss due to lower estrogen levels in the body. This leads to weaker bones that break more easily. Significant weakening of the bones may develop into a condition called osteoporosis. In order to counteract this condition and keep bones strong, women should participate in weight-bearing exercises such as walking, climbing stair, or lifting weights. Refraining from smoking and eating foods rich in calcium and vitamin D may also help. Menopause has the added risk of increasing a woman’s chances of getting cardiovascular disease. This is partly attributed to changing estrogen levels and partly due to getting older.

b. Stages of Menopause

Peri-menopause, or the time leading to a woman’s last period, begins when a woman’s ovaries start producing less estrogen and progesterone. These are hormones that regulate menstruation However, long before any symptoms occur, a woman’s fertility is already declining. By late 30s, most women’s ovaries have already started to produce less progesterone and estrogen. By the 40s, the post-ovulation spike in progesterone becomes less pronounced. Eventually, the ovaries shut down completely and a woman no longer has any more menstrual periods. Sometimes this is a gradual change, but for others it can be very abrupt. A woman whose ovaries fail earlier than expected (prior to age 45) is said to have premature menopause, which may be caused by enzyme deficiencies, chemotherapy, genetic factors, infections, or other diseases.

c. Symptoms and Signs

These symptoms may occur before and after menopause due to hormonal changes. Because every woman is different, these changes affect each woman in a different manner. These symptoms may also be an effect of aging.

  • Irregular periods

  • Changing feelings about sex
  • Forgetfulness/trouble focusing
  • Losing muscle/gaining fat or having a larger waist
  • Feeling stiff or achy

If you believe that you are nearing menopause, a physical exam as well as your medical history along with symptoms can give you a clue that you are in perimenopause.

d. Managing Symptoms:

Hot flashes:
  • Try to avoid alcohol, spicy foods, caffeine
  • Dress warmly in layers and remove clothing as you feel the flash starting
  • Use a fan
  • Take slow, deep breaths when a hot flash begins
Vaginal dryness:
  • Use a water-based, over the counter vaginal lubricant or vaginal moisturizer
  • Use vaginal estrogen creams if dryness is not sever
Problems sleeping:
  • Be physically active
  • Avoid large meals, smoking, and working right before bed
  • Establish a routine for sleeping and waking times
  • Avoid napping during the day
  • Keep bedroom dark, quiet, and cool
Mood swings
  • Try getting enough sleep and staying physically active to feel your best

  • See a therapist or join a support group
